Closed/unresolvable library issue: Rare lockup deep in audio output.

Looks like a mutex problem between libao and itself, when invoked from a callback from Apple’s CoreAudio.  Look at how libao handles this, specifically its mutexes.

Issue is outside pianod's scope.
